Integrated/Primary Care

What is Integrated Care?

Primary Care Professionals and Mental Health Experts who share the same vision, philosophy and dedication to comprehensive patient care throughout the lifespan of their clients. A holistic approach is taken to aid the patient with their physical and mental health issues. This approach takes into account the environment, educational, psychosocial, and financial factors. Integrated care is successful only due to its usage of the entire interdisciplinary team, which includes physicians, psychologists, psychiatrists, social workers, therapists, nurses and support staff. This approach provides integrated care to both youth and adult population from birth to death.

Why choose an Integrated Care practice?

Many times physicians treat and review only the physical signs and symptoms that the patient may have. They do not take into account the effect of the psychosocial factors that may be affecting the patient. Choosing an Integrated Care practice guarantees that there will be an increased collaboration between your providers, which ensures the best possible outcome for the patient.

What are the benefits?

This approach optimizes various team members’ areas of expertise. Transportation costs and issues are eased by providing numerous services under one roof. Seamless continuation of services as the patients transitions from pediatrics, to adulthood, and then to geriatrics. Improved patients compliance, cost-effectiveness in service delivery, and provider satisfaction in ensuring comprehensive care to the patients are other benefits of using an integrated care practice.
